For Your Visit

What to prepare for your visit: (COVID guidelines)

  1. Set up for CONNECT, our online patient portal, using this link.
  2. Once you've signed up for CONNECT, complete the following via your CONNECT account:
    1. Review your medical history.
    2. Update your medications.
    3. Upload a copy of your insurance card.
  3. Fill out the New Patient Intake Form or Follow Up Intake Form and email the forms to
  4. Email any pertinent medical records (i.e. imaging results, doctor's notes, etc.)
    1. If a CD is available, upload images to Ambra (see instructions here).
  5. Download the MyChart App which can be found on the Apple App Store or Google Play.

Things to Remember Day-Of Your In-Person Visit:

  1. As per hospital policy, only 1 person is allowed with you during your visit. If more than one person is present, your appointment will be cancelled.
    1. Any patient coming in for a procedure should have an escort with them, your escort must wait outside of the department during your appointment.
  2. Face coverings MUST be work at all times.
  3. E-Check in prior to your visit using these instructions.

Things to Remember Day-Of Your Virtual Visit:

  1. Make sure you are set up for CONNECT. View our TeleHealth Guidelines for further instructions on setting up for your virtual visit.
  2. Log on 15-20 minutes prior to your appointment to allow time for technical support if needed.
  3. Interpreters will be provided if you require one.

What to Know About Your First Visit:

Please arrive at least 20 minutes early to allow time to fill out your paperwork. You should expect to be here approximately 45 minutes to 1 hour the first visit to allow time for our physicians’ full evaluation and treatment.

Late Appointments: 

Patients who arrive more than 20 minutes late will be asked to reschedule their appointments.


Missed Appointments:

If you need to cancel your appointment, please cancel at least 24 hours prior to the scheduled appointment. If you fail to show up to your appointment or cancel/reschedule your apppointment 24 hrs prior, it will be considered a "no show." Patients with 2 or more "no-shows" will be discharged from the practice. 


What to Bring to Your First Visit:

In order to save time during your initial visit to the doctor, you can download and complete our medical intake forms in advance of your appointment. Please make certain to bring these completed forms to the appointment.

In order to help us serve you better, we ask that you bring to your first visit the items listed below.

  • Insurance card(s)
  • Written order from your referring doctor
  • Authorization

Many insurance companies require prior authorization from your primary care doctor to see a specialist. Patients are encouraged to contact their insurance provider to confirm whether an authorization and/or referral are required for their specific carrier. If an authorization number is required by your insurance carrier, please have your primary care physician’s office fax it to (212) 342-1470, well before your appointment date. You may verify that it has been received by contacting the Call Center at (212) 305-3535.  You should also bring it with you on the day of the appointment.

Unfortunately, in order to be seen by the specialist at the time of your appointment, you will need to have this authorization referral at the time of your visit.  If our office does not have it, you may have to pay out of pocket or reschedule your appointment.


Please bring any medical records and/or imaging with you at the time of your visit.


If you would like to request a copy of your medical records, please download, print and complete in its entirety this form.

Please fax form, along with a copy of your State Identification Card (such as valid drivers license) to 212-342-1470. Failure to comply with this protocol will delay, or even may void your request.

Please note, the retrieval of your medical records may take as long as 15 business days.
